However, because Notion Forms work differently from standard content blocks, they need to be set up and embedded correctly to appear on your published site. - Create a new Notion page and make sure it is a subpage of your parent page.
- Inside the subpage, type
/formand select Form.

- Add all the necessary questions and options to your form.

- Ensure the Notion Form is published and copy the form’s shareable link.
- Create a new separate Notion page and set it as a subpage.
- Paste the form link into the page and choose Embed.
- Confirm that the form is visible in Notion.
- Go to your Super dashboard and click Refresh to update the dashboard preview.

Unlike other blocks, Notion Forms cannot be displayed directly as part of a database or inline content in Super. To make them visible, you need to use the form’s shareable link and embed it on a dedicated page.
If the form is added without embedding, it won’t appear on your live site.
2. The form is not a subpage
For Super to detect and publish your form correctly, it must live within your site structure. This means the form should be created as a subpage of a published parent page.
